PATIENT SCHEDULING WITH APPROXIMATE DYNAMIC PROGRAMMING FOR OPTIMIZATION OF HEALTH CARE SERVICES.

 Abstract: Model that prescribes the optimal appointment date for a patient at the moment this patient makes his request at the outpatient clinic is developed. We categorized patients into two. The first category is concerned with patients with a maximum recommended waiting time. For these types of patient, the sooner these patients are scheduled the better and when the maximum recommended waiting time is exceeded, extra costs are incurred. The other category is characterized by a specific appointment time. The closer the scheduled appointment time is to the specific appointment time, the lower the costs. The objective is to minimize the long-run expected average cost. We modelled the scheduling process as a Markov Decision Process (MDP). we then apply the Bellman Error Minimization (BEM) method as an Approximate Dynamic Programming technique in order to derive an estimate of the optimal value function of our MDP of which the optimal policy (appointment date) can be derived. To determine the set of representative states, which is an element of the BEM method, we use the k-means algorithm. We test several approximation functions and find an approximation function that outperforms all other functions in the scheduling process over four, six, and eight working days. The Approximation Function B gives the near optimal appointment date for patients when appointments are requested. In general, it holds that the higher the arrival rate of patients at the outpatient clinic, the better our BEM method performs. But if the arrival rate reaches a certain value the load of the system becomes that high that it does not matter what policy is applied, since many patients have to be rejected.

ABSTRACT: Standardization and quality control are the key factors in regulating the theraputic efficacy of herbal drugs. There are various standard pharmacognostic and chemical methods for checking an identity and the quality of single herbal drugs. Present communication highlights macro and micro-morphological characteristics, powder studies, histochemical tests, micro-chemical tests, physico- chemical constants, chromatographic profile and therapeutic uses; which can be useful for checking genuiness of herbal drugs.

EFFECTIVENESS OF NLEP (NURSE LED EDUCATIONAL PACKAGE) ON ATTITUDE REGARDING PREVENTION OF OBESITY AMONG ADOLESCENTS IN SELECTED SCHOOLS

ABSTRACT: Many school-based interventions for obesity prevention have been proposed with positive changes in behavior. The objective was to assess the effectiveness of a NLEP on prevention obesity among adolescents. Schools were randomized to intervention and control group. Methods and materials: Quantitative research approach with true experimental research design was used to find out the knowledge on prevention of obesity among school adolescents. The study was conducted among 200 adolescents studying 8th, 9th, 10th 11th and 12th in selected senior secondary schools, Jalandhar, Punjab. Probability stratified sampling technique was used to select samples. Structured questionnaire was used to assess the attitude of adolescents regarding prevention of obesity. Major findings: The overall analysis shows that uncertain attitude was found in both experimental group 84% and control group 85% before the implementation of NLEP. In the experimental group, attitude scores had improved after the implementation of NLEP, i.e., positive attitude is 61%, uncertain attitude is 39% further, in the control group, It was observed that in post- test 81% had uncertain attitude, 9% had positive attitude. From the findings it can be interpreted that NLEP improved the level of attitude in the experimental group significantly than the control group. Conclusion: The results shown that, the nurse led educational package on prevention of obesity was improved the knowledge regarding prevention of obesity among the school adolescents.

Abstract: The current study examined the parental acceptance-rejection and personality assessment of sexually abused children. A sample of 50 control (25 girls and 25 boys) and 50 abused (25 girls and 25 boys) were drawn through the process of purposive sampling (N = 100). The sample consisted of school going children between the ages of 8-16. The sample was taken from non-governmental schools and NGO. Parental Acceptance-Rejection Questionnaire (PARQ-Mother/Father; Rohner, Saaverda, & Granum, 1980), Personality Assessment Questionnaire (PAQ; Rohner & Khaleque, 1978) and Emotional STROOP task, were used to explore the relationship between the variables. The results showed that girls showed greater parental rejection than boys, were less psychologically and emotionally adjusted than boys. The results also showed that boys were high on psychological abuse while girls were high on physical and sexual abuse. The results of STROOP showed that sexually abused children showed more reaction time than non-abused children.

Abstract: Multiple sclerosis (MS) is part of a spectrum of idiopathic inflammatory demyelinating diseases (IIDDs) of the central nervous system that vary from each other by lesion size and number, pathology and clinical outcome, a recent study in Najran –king khlied hospital Diagnosis for 5 years latter (2011-2017)is achieved  using consensus clinical and magnetic resonance imaging (MRI) criteria that document white matter disease disseminated in time and space .The clinical manifestations, temporal course and pathology of MS are heterogeneous, in part because it results from complex interactions of multiple genetic and environmental factors. Researcher review recent advances in understanding the incidence rate of the disease in Najran base on the genetic and environmental epidemiology and the natural history of MS. The first  incidence study in Najran provience at king kalied hospital ,Study achieved that multiple sclerosis is common in women rather than men have tendency for later disease onset with worse prognosis in the study researcher find that women who have MS in sample was 84.6% of the sample and men were 15.4% only of the sample so the findings agree with literature review  The study reveal that MS below 19 years was 38.5% of the sample have disability form  ,100% all the study cases have relapsing remitting (RRMS)multiple sclerosis type so the study findings agree with literatre review 85% of MS cases were (RRMS). The first clinical study of MS in an Arab country was in 1958 and largely reflects the practice of a neurologist  in Baghdad, Iraq. In summary, the aetiology of MS is complex and probably involves interaction between certain genetic factors and an environmental trigger(s). It is reasonable to conclude that MS is less common in Arabs than western countries,  while acknowledging that rural and remote cases, especially mild, have not been captured for Arab populations.

Abstract: Objective: Melanoma, the most dangerous form of skin cancer, develops when unrepaired DNA damage to skin cells triggers mutations that lead the skin cells to multiply rapidly and form malignant tumors. The aim of this study was to evaluate the role of Micro RNA Let-7b in melanoma cell migration. Materials and Methods: Melanoma specimens were obtained from 30 patients with skin melanoma. The human melanoma cell line A375 was used as an in vitro model for melanoma. In the present study, the wound healing assay and real-time PCR were carried out to assess the migration ability, and to determine mRNA transcription profiles, respectively. Result: Results from this study showed that let-7b expression is able to significantly inhibit A375 melanoma cell migration. The wound healing assay demonstrated that A375 cell motility increases after let-7b inhibitor treatment. Conclusion: Let-7b displayed a higher ability to inhibit melanoma cell migration and invasion, compared to other members of the Let-7 family.

Abstract: Common bean, Phaseolus vulgaris L. c.v. Shalimar Rajmash was considered as a poor nitrogen fixing plant compared to other legumes. Even though there are several attempts to produce better varieties for improved N2 fixation, no work has been done to evaluate the response of this crop to different biofertilizers under various levels of phosphorus. In order to study the effect of biofertilizers (Rhizobium, Azotobacter, Arbuscular mycorrhizae) under different levels of phosphorus (20 and 40 kg/ha) application on leghemoglobin content and enzyme activity (nitrate, nititre reductase)  in nodules of common bean  (Phaseolus vulgaris L.)  in a sustainable production system, an experiment was conducted during kharif seasons of 2012 and 2013 at the Krishi Vigyan Kendra (KVK) of Shere-e-Kashmir University of Agricultural Sciences and Technology, Budgam, Jammu and Kashmir. The climate of the experimental site is temperate with mild summers and cold winters, showing wide variations in mean maximum and minimum temperatures. Temperature varies from 5˚C in winter to a maximum of 34°C. The experiment was laid out in complete randomized block design (RBD). Different levels of   DAP and various biofertilizers namely Rhizobium (Rhizobium leguminosarum), Azotobacter (Azotobacter vinelandi), VAM (Glomus mosseae) have been used during the research. Rhizobium with VAM @ 20 kg P/ha in the present research showed significant impact on all parameters of common bean.

Abstract: Background : The aim of the study was to study the prevalence of depression and anxiety symptoms in patients with type one and two diabetes mellitus and their correlation with specific socio-demographic and clinical factors.  Methods: 428 diabetic patients participated in the present cross-sectional study. Their demographic and clinical parameters were collected. The self-administered questionnaire HADS was used to determine level of depression and anxiety. Results: We found that 22.4% of the sample experienced significant depressive symptoms and 28.5% experienced significant anxiety symptoms. It was found that: Not practicing sports (p = 0.002), smoking (p = 0.005), having lower level of education (p = 0.006) and insulin treatment (p = 0.013) were significantly associated with depression. On the other hand, female gender (p = 0.017), smoking (p = 0.007) and being unmarried (p = 0.036) were significantly associated with anxiety. Correlation showed that depression was significantly associated with BMI (p = 0.003) and age of the patients (p = 0.004). It was found that both anxiety and depression positively correlated significantly with duration of diabetes (p≤0.001). Neither depression nor anxiety was associated with HBA1c levels or with mean blood pressure. Conclusion: Depression and anxiety symptoms were high among patients with type I and II diabetes mellitus. Depression was strongly related to age, lower level of education, smoking, not practicing sports, duration of diabetes, insulin treatment and BMI. Anxiety was strongly related to female gender, smoking, duration of diabetes and being unmarried.
